Registration Procedures

Please download and print the YEP registration form, which can be found under the “Documents” section.

Once you have completed the registration form, submit it along with your child(ren)’s immunization records via email to the Enrollment Coordinator at cthrash@yep-la.org or by mailing it to 4700 Line Avenue, Suite 207, Shreveport, LA 71106. We prefer submissions in PDF format. Please avoid faxing or sending pictures of the registration forms. If you are registering more than one child, kindly include all registrations in the same email.

Once the YEP office receives your child(ren)’s registration form, you will receive a ‘Welcome’ email from the Enrollment Coordinator at cthrash@yep-la.org. Along with this, the Bookkeeper will send you an Invoice/Statement to the email address provided on the registration form. To make your payment, simply click the blue ‘Review and Pay’ button on the emailed invoice. If the site is at full capacity, you will be notified and placed on the waiting list.

YEP accepts the following forms of payment: check*, money order*, debit, and credit cards

The annual registration fees are as follows:

• 1 child: $30 ($30.71 online)
• 2 or more children: $40 ($40.94 online)

The monthly fees are:

• 1 child full-time: $162 ($165.81 online)
• 1 child part-time (3 days or less): $130 ($133.06 online)
• 2 children full-time: $314 ($321.38 online)
• 2 children part-time (3 days or less): $250 ($255.88 online)

*Please remember to include your child(ren)’s name and school on all checks and money orders.
